Do you accept insurance? expand_more
We accept most major PPO insurance plans and Medicare. As with traditional healthcare providers, we will bill your insurance directly for clinical office visits and procedures.
Please note that your standard insurance terms—including copayments, deductibles, and any non-reimbursable fees—will apply in accordance with your specific plan of benefits. If you are enrolling or seeing a non-Medicare provider; all clinical visits will be billed at our cash rate.

Is the membership fee covered by insurance? expand_more
The membership fee is an out-of-pocket expense and is not covered by insurance. It may be paid on a monthly or annual subscription basis, depending on your preference.

What is considered out-of-pocket? expand_more
Out-of-pocket expenses include your membership fee, additional visits with the registered dietitian and/or exercise physiologist beyond those included in your membership, most specialty labs and any services and products not covered by medical insurance.

How often do I see a provider? Is there a limit? expand_more
There is no limit to the number of visits you may have with the clinician. The clinician will manage your overall care plan and will recommend your appointment cadence according to your healthcare needs.
All medical visits will be billed to your PPO insurance plan. If you do not have insurance, or if you are covered under an HMO plan, you will be responsible for the visit cost as a self-pay (cash) patient.
