Do you take insurance?

Yes. We take PPO and Medicare insurance. Please refer to your individual insurance benefits to confirm your coverage benefits, co-payments and deductibles.

The membership fee is not a covered medical insurance benefit.

There may also be additional lab testing and imaging that your insurance may not cover. Our clinical care coordinator will give you more detailed information on out-of-pocket costs and services.
